What is CalAIM

The California Department of Health Care Services (DHCS) developed CalAIM as a long-term project to improve and modernize Medi-Cal. CalAIM's goal is to maximize the health and life trajectory of Californians by providing them with a more person-centered, coordinated, and equitable approach. 

 

CalAIM prioritizes prevention and whole-person care to meet members' holistic health needs. Members with complex needs can access innovative services outside of conventional healthcare settings thanks to whole-person care. For members whose wellness is influenced by complex social factors, such as homelessness, behavioral health, care needs for older adults, transitioning out of incarceration, and more, these services seek to improve health outcomes and offer comprehensive care.

Enhanced Care Management (ECM)

ECM is a brand-new initiative that supports our members with intricate social and medical requirements. In addition to traditional medical services, these new services provide coordination assistance with seven core services:

Benefits of  Being a Member of Enhanced Care Management
through My Time Movement

Dedicated Case Manager

Members will receive a dedicated Care Manager to assist them with implementation of a Care Plan, coordination with social services, advocate on behalf of the member with health care professionals, monitor treatment adherence, and provide education on the resources available to the member. 

Community Supports (CS)

Community Supports are available to Medi-Cal members to address challenges that may not involve direct medical care but significantly impact overall health. These free services are designed to support members facing complex health issues, recognizing that factors like food insecurity, unstable housing, or lack of transportation often contribute to or worsen health conditions.

My Time Movement Services Offered by CS

Housing Transition Navigation Services

Guides members through housing transitions with assessments, personalized assistance plans, and housing search support.

Housing Deposits

Provides funds for one-time expenses such as a security deposit, utility setup fees or deposits, one-time cleaning, furniture, or other items to help members set up a basic household.

Housing Tenancy and Sustaining Services

Provides funds for one-time expenses such as a security deposit, utility setup fees or deposits, one-time cleaning, furniture, or other items to help members set up a basic household.
