Comprehensive support designed to prevent homelessness before it starts
The H.E.E.N.A Assessment is a holistic, early-intervention support service designed for former foster youth ages 18–25. Our goal is simple: address unmet needs before they become crises. Through the assessment process, each participant is paired with a dedicated case manager who works alongside them to build a personalized plan toward stability, independence, and long-term success.
During the H.E.E.N.A Assessment, we identify the recipient’s needs across four core areas:
Housing, Education, Employment, and Essential Needs.

HEENA Assesment
Step 1: Connect
Reach out directly or get referred by a partner agency. Once you contact us, our team will schedule a HEENA Assessment appointment.
Step 2: Meet With Your Case Manager
You sit down with a case manager to talk about your housing situation, education plans, employment goals, health coverage, documents, and daily needs in a safe and judgment free space.
Step 3: Build Your Personalized Plan
Together, you create a clear support plan. This may include steps like securing ID, opening a bank account, applying for housing resources, setting life goals, or connecting to classes and services.
Step 4: Walk The Path Together
Your case manager stays connected, checks in regularly, and supports you as you follow through on your plan. You are not expected to do it all alone.
Who Is HEENA For
You may be eligible for the H.E.E.N.A Assessment if you:

Are between 18 and 25 years old

Have a history of foster care involvement

Want support with housing, education, employment, or basic needs

Feel like you are navigating adulthood mostly on your own
